Compassionate Hospice RN Case Manager

Gentiva

Gentiva is seeking a compassionate Hospice Registered Nurse to lead patient-centered care in patients' homes and other settings. You will develop plans of care, perform assessments, and coordinate with an interdisciplinary team to ensure comfort and quality of life. You will educate families, document care accurately, and participate in on‑call activities while upholding regulatory standards and best practices. #J-18808-Ljbffr Gentiva
