Job Description Summary

Themedical social worker is responsible for the implementation of standards ofcare for medical social work services. The medicalsocial worker provides direct and indirect social worker services primarily ona visiting and intermittent basis in the patient's home.


Essential JobFunctions/Responsibilities

1. Assesses the psychosocial status of patientsrelated to the patient's illness and environment.

2. Carries out social evaluations and plansintervention based on evaluation findings.

3. Maintains clinical records on all patientsreferred to social work.

4. Provides information and referral servicesfor patients and families/caregivers regarding practical and environmentalneeds.

5. Provides information to patients orfamilies/caregivers and community agencies.

6. Serves as liaison between patients orfamilies/caregivers and community agencies.

7. Maintains collaborative relationships withagency personnel to support patient care.

8. Maintains and develops contacts with publicand private agencies as resources for patient and agency personnel.

9. Participates in the development of the totalplan of care and case conferences as required.

10. Assists physician and other team members inunderstanding significant social and emotional factors related to healthproblems.

11. Participates in discharge planning.

12. Other duties as delegated by the ClinicalDirector/supervisor.

Position Qualifications

1. Graduateof a Master's program in social work accredited by the Council on Social WorkEducation.

2. Minimumof one year's experience in health care.

3. Experiencein a home health care preferred.

4. Demonstratesgood verbal and written communication, and organization skills.

5. Possessesand maintains current CPR certification.

6. Must have a valid driver license and auto insurance.
