All activities in the last three (3) years must be verifiableMeet or exceed the medical standard of DOT, including passing a physical exam and drug test administered by a company approved facilityPossess a valid Social Security CardNo reckless driving violations in the last ten (10) yearsNo suspension of license in the last ten (10) yearsNo moving violations in the last three (3) years – No more than one (1) moving violation in the last five (5) years – No at fault accidents within the last five (5) years – No more than two (2) moving or non-moving violations in a five (5) year period. – 1 -3 years no more than one (1) moving violation. 2 -5 years no more than 2 moving violationsNo failed or refused drug or alcohol testNo felony convictions within the last ten (10) years from the time restitution was completed.
