Implement evidence-based interventions to improve gait, transfers, activities of daily living, and overall mobility. Reassess resident progress regularly, adjust goals, and document outcomes in required health records. Collaborate with an interdisciplinary team (nurses, therapy aides, and other therapists) to coordinate care and optimize outcomes. Provide patient and family education, discharge planning input, and home exercise programs to support continued progress after therapy. Ensure adherence to safety, infection control, and privacy standards while delivering high-quality care.
