RN Case Manager

Overview Job Type: Full-Time Schedule: Monday–Friday from 8:00 AM to 4:30 PM Location: Eagan, MN Job Description: Regency Home HealthCare has immediate opportunities for compassionate people to join our team! This position provides a combination of direct care by performing hands-on nursing, advocating for, and collaborating with clients and their families, and keeping communication open with the entire healthcare team. Represent the company’s values while maintaining a professional working relationship with others. Perform a combination of hands-on care for medically complex clients on skilled nursing visits. Advocate for clients and assist them in achieving their goals and independence. Follow all guidelines and remain in compliance with documentation and policies.
