Physical Therapist (PT) - Outpatient

Physical Therapist Rehabilitation

8-hour day shifts.

Primarily inpatient and outpatient care, as needed. Minimum of 2-3 years of experience required. Hospital swing bed program.

Requirements

• Active Physical Therapist license
• Experience with EMR systems, specifically Web PT and TruBridge
• Familiarity with swing bed programs

Responsibilities

• Evaluate and treat patients with various physical therapy needs
• Develop and implement individualized treatment plans
• Collaborate with other healthcare professionals to optimize patient care
• Document all patient interactions and progress in EMR system
• Provide education and support to patients and their families
