Occupational TherapistThe Occupational Therapist will assess, plan, organize, and participate in rehabilitative programs that help build or restore vocational, homemaking, and daily living skills, as well as general independence, to persons with disabilities or developmental delays.Essential Duties And ResponsibilitiesThe essential functions include, but are not limited to the following:Develops, implements, and evaluates the plan of care in partnership with the client, representative, caregiver(s), and physician(s)Provides direct occupational therapy services in accordance with the plan of care established by the physicianEducates and instructs client, family members, or other client representatives, in rehabilitation care and activities necessary to promote the client's health, safety, and independent livingAccurately documents observations, interventions, and evaluations pertaining to client care management and services providedSelect activities that will help individuals learn work and life-management skills within the limits of their mental or physical capabilitiesRecommend changes in patients' work or living environments, consistent with their needs and capabilitiesEngage in research activities and participation in formal educational training activities related to therapy.Perform other duties as assignedMinimum Qualifications (Knowledge, Skills, And Abilities)Current and valid Connecticut Occupational Therapist license (required)Recent work experience in home health (preferred)Driver’s LicenseCar InsuranceCar RegistrationStrong clinical assessment, organizational, and communication skillsAbility to handle multiple assignments simultaneouslyAbility to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentationBilingual (Spanish) preferredExcellent oral and written communication skillsExcellent customer relationship skillsPhysical Demands And Work EnvironmentThe physical demands described here are representative of those that must be met by an employee to perform the essential functions of this position successfully.
