Let's talk about the Care Manager position! This full-time, hybrid position combines remote work from your home office with travel of up to one hour for community-based meetings several times a week. Expense and mileage reimbursement What You’ll Do: Provide information and education to help Individuals understand the Medicaid waiver process Create an individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve Help Individuals access services and supports they need to meet their goals Facilitate regular community‑based team meetings to review plan of care and collaborate with service providers as needed Who You’ll Work With: Case Managers work with Individuals on the Health & Wellness (H&W) and Traumatic Brain Injury (TBI) Medicaid Waivers What You Need: A valid driver's license, car insurance, and reliable transportation RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services The ability to pass thorough background screens Designated home office setting with computer, internet service, and smart phone #J-18808-Ljbffr Indiana Professional Management Group
