Full-Time CNA / Patient Care Coordinator (PCC)

Patient First Hospice & Palliative Care is seeking a dependable, organized, and compassionate Certified Nursing Assistant (CNA) / Patient Care Coordinator (PCC) . This is a full-time, primarily office-based position, Monday–Friday, 8:00 AM–5:00 PM , with CNA field support provided as needed.

The PCC supports daily patient care coordination and administrative operations, including scheduling, communication with patients, families, facilities and clinical staff, maintaining patient records, processing orders and documentation, managing calls, emails and faxes, and assisting with general office operations.

Key Responsibilities:

• Coordinate patient visits, schedules, admissions, discharges, and follow-up needs.
• Maintain accurate patient information and documentation in the EMR.
• Manage telephone calls, emails, faxes, medical records, and office correspondence.
• Coordinate with clinical staff, physicians, facilities, pharmacies, DME providers, and other vendors.
• Assist with physician orders, patient supplies, documentation tracking, and administrative tasks.
• Provide professional and timely communication with patients, families, and the interdisciplinary team.
• Provide CNA patient care in the field when needed.
• Maintain confidentiality and comply with HIPAA requirements.

Qualifications:

• Current CNA certification in good standing.
• Strong administrative, organizational, and communication skills.
• Proficiency in Microsoft Office, Microsoft Word, and Outlook required.
• Healthcare administrative, hospice, home health, or medical office experience preferred.
• Comfortable using electronic medical records and office technology.
• Dependable, punctual, detail-oriented, and able to multitask.
• Valid driver's license, reliable transportation, and current automobile insurance required .
• Ability to work independently and collaboratively in a fast-paced healthcare environment.

This position is ideal for an experienced CNA interested in expanding their skills in patient care coordination, healthcare administration, and hospice operations while continuing to support patients directly when needed.
