LICENSED PRACTICAL NURSE

Summary This Licensed Practical Nurse position is located within the Primary Care Service at the VA Clinic in Middletown, Rhode Island. This is a full‑time, 40‑hour per week position. Qualifications United States Citizenship (non‑citizens only under exceptional circumstances) English Language Proficiency; proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d) Full‑active‑current and unrestricted licensure as a licensed practical or vocational nurse in a U.S. state, territory, Commonwealth or District of Columbia Graduate of a school of practical or vocational nursing approved by the National League for Nursing Accrediting Commission Or military health‑care education accepted by the licensing body as qualifying for full LPN/LVN licensure Or VHA employee conversions under 38 U.S.C. 7401(3) or 7405, with no subsequent waiver Or license granted by a jurisdiction that does not require graduation from an approved school May qualify under the Grandfathering Provision of the VA Qualification Standard (for current VHA employees only) Grade determination GS‑06: Completion of at least one year of qualifying experience at the GS‑5 level or equivalent Technically proficient in initiating, performing, and completing assigned duties in patient care Knowledge and ability to appropriately carry out patient care based on patients’ conditions Judgment in selecting appropriate order and sequence of procedures and treatments Accurate recognition, reporting, and recording of relevant patient information Can work independently with general review by RN or MD/DO Can observe, identify, and respond to patient needs for medication, equipment, and family education Recognizes emotional, socio‑economic, and age‑related factors in care Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular, and/or intravenous) and performs treatments per policy Observes for changes following treatments, documents accurately, and reports deviations to RN or MD/DO Recognizes urgent or emergent situations and initiates appropriate interventions Uses knowledge of human behavior to work effectively with patients, families, and staff Establishes constructive relationships with patients and families to promote positive communication and socialization skills Fosters respect for patient and family rights to privacy and dignity Incorporates customer service standards in all interactions Performs support duties for complex diagnostic tests and specialized procedures, including preparing the patient and handling instruments Serves as a preceptor for LPNs/LVNs or NAs/HTs in support duties Actively seeks educational opportunities to enhance nursing knowledge and shares new knowledge with staff Preferred: Primary Care Service experience Reference: For more information on this qualification standard, visit Physical Requirements: Refer to VA Handbook and Directive 5019 – Employee Occupational Health Duties Deliver direct patient care to adult through geriatric patients Recognize and apply age‑related, cognitive, emotional, and chronological needs in care Actively participate in the healthcare team, supporting interdisciplinary treatment plans Attend inter‑disciplinary team meetings and contribute to comprehensive treatment plans Vary order and sequence of assigned procedures based on patient condition knowledge Assume responsibility for patients under TJC Standards, with ultimate RN responsibility Identify environmental hazards and initiate appropriate intervention Collect data and collaborate in assessing patient health status Implement delegated aspects of the plan of care Participate in health screenings, clinical reminders, telephone calls, and treatments as assigned Evaluate patient responses to interventions Promptly recognize, report, and record changes in physical condition and behavior Complete routine documentation of care results per policy and electronic records Resolve ethical issues through proper channels Establish and maintain positive interpersonal relationships with patients, visitors, and personnel Communicate and collaborate with other healthcare professionals as delegated Work schedule: Monday to Friday, 8:00 a.m. – 4:30 p.m. Not authorized for compressed/flexible schedule, telework, or virtual work Not authorized for recruitment incentive, relocation assistance, or PCS Competitive salary with regular salary increases Paid time off: 37–50 days per year (13–26 days annual leave, 13 days sick leave, 11 paid federal holidays);
