Hospice Inpatient Liaison RN - Care Coordination

VIA Health Partners

VIA Health Partners is seeking a Registered Nurse in Charlotte, North Carolina, to provide direct inpatient care and serve as a liaison with hospital systems. This role requires a current NC/SC RN license and a minimum of two years’ related experience, particularly in hospice and palliative care. The ideal candidate will ensure accurate clinical documentation, maintain professional accountability, and engage in direct patient communication.
