Job Description
TITLE: Registered Nurse - Home Health

SCHEDULE: 8 hours

ASSIGNMENT DETAILS:
- Contract Length: 14 weeks
- Guaranteed Hours: 40 per week

REQUIREMENTS:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance

ROLES & RESPONSIBILITIES:
- Assess patients in home health and/or hospice settings to identify physical, psychosocial, and environmental needs.
- Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and families regarding care, medications, and home safety, using age-appropriate teaching methods.
- Supervise and provide clinical direction to home health aides and other support staff.
- Manage pain through assessment and utilization of effective techniques, educating patients about pain management.
- Facilitate communication among patients, families, and the interdisciplinary team to coordinate care effectively.

COMPLIANCE / NOTES:
- Must meet mandatory continuing education requirements.
- Maintains confidentiality as required.
