Job Summary
Outpatient Physical Therapist (PT OP) located in an outpatient setting, this position involves evaluating patients, developing and implementing individualized treatment plans, and ensuring effective return to community activities. The role requires collaboration with patients, families, and healthcare teams to deliver high-quality therapy services that align with organizational and safety standards.

Job Details

• Position Description: Evaluates assigned patients and develops, implements, and modifies treatment plans based on patient needs, goals, and physician directives. Responsible for delegating and directing all discipline-specific therapy services for assigned patients. Ensures the design of functional treatment programs promotes effective community reintegration. Serves as an educator for patients, families, students, and staff. Performs other professional duties associated with patient care, discipline-specific therapy development, and program delivery. Upholds organizational values including safety, quality, professionalism, and confidentiality.
• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Degree in Physical Therapy from an Accredited School; 1+ Year of Outpatient Clinical Experience; CPR Certification.
• Schedule: Days shift, 8 hours per shift, minimum of 40 hours per week.
• Additional Information: Completes comprehensive evaluations, develops treatment goals geared toward progressing patients towards discharge within an established length of stay, and ensures documentation in compliance with regulations and clinic policies. Participates in student programs, pursuing ongoing professional development, and maintains licensure in good standing. Maintains adherence to regulatory and safety standards and contributes to organizational performance improvement initiatives.
