Responsibilities Provide direct patient care including wound care, medication administration, and vital sign monitoring Develop and follow individualized patient care plans in coordination with supervising RNs and physicians Observe and document patient health status, progress, and any changes in condition Assist patients with activities of daily living (ADLs) and personal hygiene as needed Educate patients and family members on care procedures, medications, and health management Communicate effectively with the healthcare team to ensure continuity of care Maintain accurate and timely clinical documentation in compliance with state and agency regulations Requirements Active Licensed Practical Nurse (LPN) license in the state of Georgia Prior home health, long-term care, or clinical nursing experience preferred Strong knowledge of nursing procedures, medication management, and patient assessment Excellent communication, organizational, and time-management skills Ability to work independently and exercise sound clinical judgment Valid driver's license and reliable transportation required CPR/BLS certification preferred About Us Presidential Home Care LLC is a trusted home health care provider serving the Norcross, GA community and surrounding areas, committed to delivering compassionate, patient-first care where it matters most — at home.
