Hours Shift Start Time: 8 AM Shift End Time: 4:30 PM AWS Hours Requirement: 8/40 - 8 Hour Shift Additional Shift Information: Some days 9 am to 5.30 pm Weekend Requirements: Every Other On-Call Required: No Hourly Pay Range (Minimum - Midpoint - Maximum): $32.730 - $40.910 - $45.810 The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. What You Will Do To provide documentation for discharge planning, psychosocial, treatment plan, UR review, and care coordination for patients. Intake phone calls and assessment patient for appropriate programs. Required Qualifications Bachelor's Degree In Social Work. AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) -REQUIRED Preferred Qualifications Hospital experience. Case management experience.
