Superior Senior Care – Pine Bluff is looking for a compassionate, organized professional to join our team as a Targeted Case Manager (TCM)!
If you're passionate about helping individuals access the services they need while making a meaningful impact in your community, we'd love to hear from you.
✅ What You'll Do• Conduct client assessments and develop individualized care plans
• Coordinate community resources and support services
• Monitor client progress and maintain accurate documentation
• Advocate for clients and help them remain as independent as possible
✔ QualificationsWe're seeking candidates who meet Arkansas Targeted Case Management requirements, including:
• Licensed Social Worker (LMSW or LCSW), or
• Bachelor's degree in a qualifying health or human services field
• Excellent communication and organizational skills
• Reliable transportation and willingness to travel within the service area
• Case management experience is a plus
