Patient Care Coordinator RN, Holy Cross Hospital

About This Role The Patient Care Coordinator RN is responsible for managing and coordinating care for the acute inpatient population, collaborating with healthcare teams to ensure effective patient care progression and safe discharges. Highlights Pay: $48 - $63/hour Location: Silver Spring, MD Facility: Holy Cross Hospital Schedule: Full-time, Day shift What You'll Do Complete an initial face-to-face assessment for every admitted member within 24 hours of admission. Document assessment outcomes and communicate with the healthcare team and patient/family. Participate in daily Care Without Delay (CWD) rounds reporting on patient progression. Review and document discharge plans in accordance with policies and regulatory requirements. Document updates, care progression, and barriers to discharge daily. Manage timeliness of care progression with physician and nursing staff. Collaborate with Social Worker to coordinate long-term care and other services. Send referrals to in-network vendors for post-acute levels of care. Identify, record, and escalated delays in care and barriers to discharge. Schedule follow-up appointments and referrals for high-risk patients prior to discharge. Requirements Education Associate Nursing degree required. Experience Minimum of one (1) year in an acute medical/surgical/ED or critical care nursing area. Licenses & Certifications Registered Nurse License (Virginia) within 6 months of hire AND Registered Nurse License (Maryland) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire. Registered Nurse License (District of Columbia) within 6 months of hire. Basic Life Support certification required.
