Role: Licensed Occupational Therapist (OT) providing PRN (as-needed) rehabilitation services in a skilled nursing and rehab facility. Responsibilities: Evaluate and treat geriatric patients with complex medical, cognitive, and functional needs; develop individualized treatment plans to enhance mobility, daily living skills, and independence; collaborate with interdisciplinary teams including nurses, physicians, speech therapists, and physical therapists to deliver coordinated, patient-centered care; provide adaptive equipment training, fall prevention strategies, and patient/caregiver education; ensure high-quality, compassionate care across variable schedules and patient acuity levels. Key Duties: Plan, organize, develop, and implement occupational therapy services in compliance with federal, state, and local standards; maintain documentation; work independently and within team environments; adapt to different patient needs and work pressures. Qualifications & Skills Current NY license and registration as an OT; current malpractice insurance; license in good standing. Energetic, enthusiastic, compassionate, patient, and flexible; excellent communication, organization, and customer service skills. Ability to work independently and collaboratively; adapt to varying schedules and patient needs; committed to quality care.
