Job Description
Title: CNA - Skilled Nursing Facility

Schedule:
Shift: 1500-2330
Every other weekend required.

Assignment Details:
Contract Length: 13 weeks
Guaranteed Hours: Not specified

Requirements:
- Minimum of 2 years of experience as a CNA
- BLS certification
- Current NYS license required

Roles & Responsibilities:
- Provide direct patient care and assist with daily living activities.
- Monitor patient conditions and report changes to appropriate personnel.
- Maintain accurate patient records and documentation.
- Collaborate with healthcare team members to ensure quality care delivery.
- Adhere to safety and infection control protocols.

Compliance / Notes:
- Local travelers within 50 miles are accepted with a decreased bill rate.
- The facility has the right to cancel up to 5 shifts at no cost during the duration of the assignment.
