Requirements

• Required certification: Physical Therapist
• 1+ years of experience
• Key Requirements Summary: 1. Experience: 2 years required for submission 2. Cases: Outpatient PT setting 3. Certifications: BLS required; all certs must be AHA or American Red Cross and in hand at time of submittal; copies required at submission 4. License: CA License required; valid state license required at submission; Nursys within last 2 weeks 5. Education: Graduate of accredited Physical Therapy school approved by American Physical Therapy Association with B.S., B.A., M.A., M.S., or D.P.T.
