Duties

The Nurse Practitioner is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Major duties and responsibilities of the position:

• Serve as primary advanced practice provider for short-stay behavioral health residents.
• Perform comprehensive medical evaluations, admission and discharge planning, including different diagnosis, risk assessments, and ongoing symptom monitoring.
• Initiate and manage non-pharmacologic treatments based on geriatric best practices.
• Lead behavioral crisis prevention and intervention strategies using trauma-informed, culturally sensitive care models.
• Coordinate with Psychiatry, Psychology, Social Work, Nursing, Rehab Therapies, and community partners to ensure continuity of care.
• Lead and continually refine the Short-Stay Behavioral Care Services Program, ensuring alignment with CLC short-stay standards, mental health integration requirements, and VHA directives.
• Develop, maintain, and oversee workflows, care pathways, and interdisciplinary team processes.
• Support accreditation and regulatory readiness, enduring documentation meets CMS, Joint Commission, and VHA expectations.
• Track performance metrics, identify improvement opportunities, and led qualify improvement initiatives.
• Facilitate team huddles, care plan conferences, and complex case discussions.
• Serve as the primary clinical consultant on medical issues for CLC nursing and support staff.
• Provide resident and family education on medical symptom management, treatment expectations, and discharge readiness.
• Develop and deliver ongoing staff education on medical assessment, geriatric health trends, and care standards.
• Mentor nursing staff in mental health competencies, clinical reasoning, and person-centered behavioral approaches.
• Establish clear discharge goals at admission and ensure coordinated discharge plans with Mental Health, Primary Care, and community services.
• Support safe transitions with comprehensive documentation, medication reconciliation, and follow-up coordination.
