Key Responsibilities

• Conduct thorough health assessments, formulating diagnoses for both acute and chronic medical conditions encountered by our residents.
• Collaborate with physicians and other health professionals to create and implement evidence-based treatment plans tailored to individual patient needs.
• Order and interpret necessary diagnostic tests, managing the appropriate diagnostics and treatments to enhance patient recovery.
• Educate patients and their families about conditions, treatments, and post-discharge caregiving, ensuring they are well-informed about their health management.
• Engage in regular interdisciplinary meetings, sharing insights on residents’ progress and challenges to refine ongoing care strategies.
• Document patient interactions accurately and promptly within our electronic health records (EHR) system, maintaining up-to-date patient charts that reflect care provided.
• Participate in quality improvement initiatives by tracking health outcomes and implementing evidence-based practice guidelines to enhance the quality of care.
• Stay current with medical advancements and regulatory requirements relevant to assisted living and nursing practice.

Patient Population / Client Focus

The primary focus of this role centers on elderly residents, many of whom may exhibit complex health conditions that require nuanced and compassionate care approaches. You will encounter a diverse patient population, from those with developmental disabilities to chronic illnesses requiring ongoing assessment and medical management. Your work will play a crucial role in providing these residents not only the medical care they require but also the emotional and social support they need to thrive.

Qualifications

• Active certification as a Nurse Practitioner.
• Current and valid nursing license for the state of Georgia.
• Bachelor’s degree in Nursing or higher from an accredited institution.
• A minimum of 2 years of relevant clinical experience in an assisted living, skilled nursing, or clinical setting.
• Proficiency in verbal and written communication, alongside strong interpersonal skills for effective collaboration.
• Demonstrated critical thinking abilities and problem-solving skills when addressing complex patient needs.

Preferred Experience

• Experience within an assisted living or long-term care environment is highly desirable.
• Familiarity with computerized medical records systems and documentation practices.

Compensation and Benefits

We offer a competitive salary range of USD 120,000 to 220,000, commensurate with your qualifications and experience.
