Growing together.As a Hospice Admissions Nurse, you will be responsible for executing the hospice admission process in compliance with federal and state regulations. You will advocate for the patient and family to meet their needs that hospice can cover and refer them to other resources as appropriate.Primary Responsibilities:Demonstrates competency with potential patients, hospice eligibility and meeting with families in assessment of their needsMaintains confidentiality and safety of patient/client clinical recordsCompletes assigned admissions and patient care orders in an effective and timely mannerCommunicates effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding the new admission of a patient. Records that may be relevant to determine hospice eligibilityCommunicates effectively with operational, clinical and sales leadership regarding status of pending referralsYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Current unrestricted RN licensed in the state of operation2+ years of experience in a hospice, home health, or other healthcare service delivery system settingCurrent Driver's License and vehicle insurance, and access to a dependable vehicle or public transportationState Specific Requirement:MS: At least one-year full-time experience as an RN. However, three (3) years full-time clinical experience in a healthcare setting as an LPN may be substituted for the one-year full time experience as an RNPreferred Qualifications:Current CPR certification or ability to complete within 90 days of hireDemonstrated fiscal responsibility and accountabilityProficient computer skills with knowledge of Microsoft Word and ExcelAt UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
