HC - Nursing (Ambulatory Local + Per Diem) WCS Healthcare is seeking an experienced and compassionate Lead RN Care Coordinator to oversee the delivery of comprehensive health services for children in New Haven, CT. Job Description: Compensation : $ 40 Job ID : 45467 Job Function: Day Shift. 8:30 AM – 5:00 PM. Oversee and coordinate healthcare services for children, ensuring timely and appropriate medical, dental, and behavioral health support Supervise and mentor care coordination staff, providing day-to-day guidance and oversight Develop and deliver staff training on healthcare protocols, public health practices, and compliance standards Coordinate interpretation and translation services to ensure equitable access to care Support care transitions by coordinating post-program or post-release healthcare services Collaborate with healthcare providers, agencies, and community resources to ensure continuity of care Monitor compliance with healthcare regulations and organizational policies Maintain accurate, timely, and compliant medical documentation Participate in interdisciplinary meetings to support holistic care planning Job Requirements: Job Requirements: Active Registered Nurse (RN) license in Connecticut Bachelor’s degree in Nursing or higher Minimum of 2+ years of clinical experience (pediatrics, community health, or care coordination strongly preferred) Strong knowledge of care management principles and community resources Excellent communication and organizational skills Ability to work independently while collaborating within a team environment Prior supervisory or leadership experience in a healthcare environment Bilingual in English and Spanish (required) Disclosure: The hourly rates and/or salaries listed may or may not reflect total compensation packages including bonus and fringe benefits, etc., nor are the advertisement(s) posted a guarantee of a certain compensation package for a position or bona fide offer of employment.
