Medical Social Worker (MSW) - Home Health

Overview Sequoia Home Health and Hospice is seeking an experienced and compassionate Medical Social Worker to join our team and provide individualized care to home health and hospice patients. Job Summary The Medical Social Worker is responsible for the implementation of standards of care for medical social work services. Responsibilities The Medical Social Worker assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with patient, representative, and caregiver(s). Assesses the psychosocial status of patients related to illness and environment and communicates findings to the registered nurse. Carries out social evaluations and plans intervention based on evaluation findings. Provides patient, caregiver, and family counseling. Assists physician and other team members in understanding significant social and emotional factors related to health problems. Provides information and referral services for patients and families/caregivers regarding practical and environmental needs. Provides education to patients or families/caregivers and community agencies. Serves as liaison between patients or families/caregivers and community agencies. Maintains collaborative relationships with personnel to support patient care. Maintains and develops contracts with public and private agencies as resources for patients and personnel. Requirements Master of Social Work degree. Preferred: one year of social work experience in a health care setting, with preference for home health care experience. Demonstrated verbal and written communication and organizational skills. Current CPR certification. Licensed driver with an insured automobile that meets state or organization requirements and is in good working order. Benefits Pay Range: $40 - $45 per hour.
