Reporting and accountable to the Nurse Manager/Director, the Clinical Nurse is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting. The Clinical Nurse participates in identifying and meeting personal learning needs. They attend education programs based on identified learning needs. The Clinical Nurse uses patient education materials relevant to the patient population with assistance. They assess the readiness to learn of the patient, family, and caregivers. The Clinical Nurse evaluates the developmental level of the patient and factors affecting their ability to learn. They identify expected patient outcomes and seek guidance as needed in revising the plan of care. The Clinical Nurse participates in discussions with interdisciplinary team members in the evaluation of patient care. They communicate relevant information to promote continuity of care. The Clinical Nurse demonstrates competence in caring for patients with complex problems, including population-appropriate physical, psychosocial, educational, and safety aspects of care. They perform bedside point of care testing as required for the patient care plan. The Clinical Nurse utilizes healthcare organization and nursing standards, policies, and procedures in the delivery of care. They organize and prioritize care according to patient and family needs. The Clinical Nurse consults with appropriate resources in a timely manner regarding patients with complex care issues, unusual teaching needs, and those at high risk for discharge planning. They develop goals to promote professional growth or minimize limitations with assistance. The Clinical Nurse achieves goals and objectives within the identified time frame or renegotiates with the supervisor. They are knowledgeable about activities that facilitate intra/interdepartmental collaboration. The Clinical Nurse participates in the development and achievement of unit goals and performance improvement activities. They effectively use communication systems and participate in promoting cost-effective care. The Clinical Nurse provides feedback to coworkers and is aware of public policy and regulatory guidelines affecting the healthcare environment. They promote a safe, clean, and secure hospital environment for all. The Clinical Nurse recognizes data from complex situations to determine priorities for care, including physical, psychosocial, education, and safety needs. They synthesize assessment data into a meaningful whole prior to communicating with others. The Clinical Nurse assesses and anticipates discharge needs of individual patients and families. They develop relationships with families that promote their ability to advocate for both the patient and their own needs. The Clinical Nurse collaborates with patients and families to prepare or update the plan of care. They utilize available multidisciplinary resources in planning care. The Clinical Nurse begins to use a full range of communication methods to convey planning. They identify and address cultural and ethnic issues in planning patient care. The Clinical Nurse demonstrates the ability to prioritize tasks for patients with complex problems. They read journals that contain studies or articles that may be applicable to practice.
