About the Role
We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.Clinical Responsibilities

• Provide skilled physical therapy services according to established plans of care.
• Deliver therapeutic exercise, gait training, balance activities, transfer training, and mobility interventions.
• Monitor patient progress and modify treatment plans as needed to achieve functional goals.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents and caregivers on home exercise programs, fall prevention, and safety strategies.
• ​​​​​​​Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering compassionate, high-quality therapy services.

Director of Rehabilitation Responsibilities

• Act as the main liaison between leadership and the facility.
• Oversee staff performance and support professional development.
• Manage administrative and operational tasks across therapy services.
• Coordinate meetings, training, and events for staff and residents.
• Maintain communication with residents, families, and leadership.
• Drive referrals for all disciplines

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
• Previous leadership, supervisory, or Director of Rehabilitation experience preferred, but leadership potential will be considered
• Strong communication, organizational, and leadership skills
