Home Health Nurse

The Home Health Nurse provides comprehensive patient care within the home setting, utilizing evidence-based practices to meet individual patient needs. This role involves coordinating patient care plans, managing case loads, and ensuring compliance with regulatory standards.

Responsibilities:

• Assess patient and caregiver needs using the nursing process to develop individualized care plans.
• Develop and communicate realistic, evidence-based care plans that include patient and caregiver input.
• Serve as Case Manager, actively managing caseloads and visit schedules to meet productivity standards.
• Complete and maintain accurate OASIS documentation complying with agency policies and federal regulations.
• Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective, high-quality patient care.
• Stay current on high-tech skills such as IV therapy, wound VAC, and complex wound care, ensuring infection control standards are maintained.

Qualifications:

• License as a Registered Nurse (RN) in good standing.
• Proven experience in home health nursing and familiarity with OASIS documentation.
• Knowledge of evidence-based nursing practices and federal/state regulations.
• Strong communication skills to collaborate with interdisciplinary teams.
• Ability to assess patient needs, develop care plans, and evaluate patient responses.
• Demonstrated proficiency in wound care, IV therapy, and infection control procedures.
