Care Manager (Case Management) - FS II

Thisposition reports to the Director, Case Management the Care Manager position isto support the physician and interdisciplinary team in facilitating patientcare, with the underlying objective of enhancing the quality of clinicaloutcomes and patient satisfaction while managing the cost of care and providingtimely and accurate information to payers. The role integrates and coordinatesutilization management, care facilitation and discharge planning functions. TheCare Manager is accountable for a designated patient caseload and planseffectively to meet patient needs, manage the length of stay, and promoteefficient utilization of resources. In addition, the Care Manager helps drivechange by identifying areas where performance improvement is needed (e.g., dayto day workflow, education, process improvements, patient satisfaction) . * All Flexible Schedule II employees will work two (2) shifts in a pay period. Unit needs may warrant more frequent use. Ability to meet all job & physical requirements as outlined in job description or as agreed through a work place accommodation. EDUCATION/TRAINING/EXPERIENCE: BS degree in nursing from an accreditedschool of nursing preferred. Three or more years strong clinicalexperience in clinical practice area to which assigned preferred. Strong analytical, data management, andcomputer skills desired. CERTIFICATIONS/LICENSES: Current California State License as aRegistered Nurse required. Current CPR certification required. Case Management (ACM or CCM)certification preferred. MAB certification required within 3months of hire if assigned to the Emergency department. #J-18808-Ljbffr Redlands Community Hospital
