Humana's Primary Care Organization is seeking a full-time Float Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at our clinic in Orlando, FL.

As a Nurse Practitioner (NP) or a Physician Assistant (PA) at CenterWell Senior Primary Care, you will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and moreall working together to deliver personalized, high-quality care.

You will report to the Associate Medical Director and float as needed throughout our Orlando, FL market.

Your Role & Responsibilities

• You will provide comprehensive care to seniors following clinical standards.
• You will collaborate daily with your care team to ensure coordinated, high-quality care.
• You will manage referrals, hospital/SNF coordination, DME, and home health services.
• You will document care with support from quality-based coders.
• You will participate in on-call rotation and support growth plans at your center.
• You will have experience with providing value-based care.
• You will work on-site.

What We're Looking For

Required:

• Master's degree in nursing with board certification, or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of Florida.
• At least one year of experience in value-based care, primary care, or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Scheduled Weekly Hours: 40

Pay Range: $123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
