Recovery Case Manager: Substance Use & Aftercare

Bay Cove Human Services in the Greater Boston area seeks a Case Manager for Residential Recovery Services to provide counseling, support, and referrals to individuals with substance use and co-occurring disorders. You will coordinate services, develop aftercare plans, document in the Electronic Health Record, and work with a clinical team on discharge planning and relapse prevention.
