Key Responsibilities\n- \n
- Conducting initial assessments and ongoing evaluations to identify client needs and collaboratively set achievable treatment goals. \n
- Providing evidence-based, prevention-oriented therapy designed to empower clients and facilitate positive change in their lives. \n
- Creating, implementing, and reviewing individualized treatment plans to ensure they are in line with clients' evolving needs. \n
- Actively collaborating with other healthcare providers, social workers, and placement agencies to coordinate comprehensive care for clients. \n
- Ensuring all documentation, including notes and treatment updates, is completed accurately and in a timely manner to support continuity of care. \n
- Maintaining a manageable caseload, conducting regular client sessions, and ensuring thorough session notes are kept for each meeting. \n
- Participating in weekly staff meetings to discuss client needs, share resources, and foster team cohesion. \n
- Contributing to a positive organizational culture by sharing insights and participating in professional development opportunities. \n
You will primarily serve clients from various backgrounds, including adults with anxiety, depression, trauma, and other mental health conditions. We also focus on underserved communities, emphasizing the importance of culturally competent care. Your role will be pivotal in helping these individuals navigate their challenges and improve their overall mental health.
Qualifications\n- \n
- Must hold a current and valid license as a Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Independent Clinical Social Worker (LICSW). \n
- A Bachelor’s Degree in Psychology, Social Work, Counseling, or a related field is required; a Master’s Degree is preferred. \n
- Proficient knowledge of mental health assessments, intervention strategies, and treatment modalities. \n
- Strong interpersonal skills with the ability to establish rapport and trust with clients. \n
- Exceptional written and verbal communication skills, essential for effective documentation and collaboration. \n

- Previous work experience in a community health setting or a similar mental health environment. \n
- Familiarity with utilizing electronic health record (EHR) systems for documentation and case management. \n
- Experience working with diverse populations and understanding cultural nuances in mental health treatment. \n
- Demonstrated leadership capabilities, enabling a collaborative approach to mental health care. \n
