Occupational Therapist (OTR) - Home Health | Grove City, PA | $10K First-Year Success Bonus

Our client is seeking a compassionate and skilled Occupational Therapist (OTR) to join their extraordinary home health care team in Grove City, Pennsylvania and surrounding areas. This position is eligible for a $10K first-year success bonus , awarded for achieving key milestones during your first year.

Required Qualifications

Candidates must meet ALL of the following:

Education & Experience

• Bachelor's degree or higher in Occupational Therapy
• 1+ year of experience in Occupational Medicine
• 2+ years of experience in Social Work
• 1+ year of healthcare experience

Licensure & Certifications

• Valid Occupational Therapist, Registered (OTR) license in Pennsylvania (PA)
• Valid Driver's License (DL)
• ANY of the following valid certifications:
  • American Heart Association Basic Life Support (AHA BLS)
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
  • American Heart Association Pediatric Advanced Life Support (AHA PALS)

Key Responsibilities

• Collaborate within a team-based care model to manage and deliver patient care plans
• Provide home-based occupational therapy services to patients in Grove City and surrounding communities
• Evaluate patient needs and develop individualized treatment plans
• Support patients and families through education, therapeutic interventions, and care coordination
• Meet critical charting deadlines and maintain accurate EMR documentation
