The Supportive Housing Case Manager will provide recovery support services to clients with severe and persistent mental illness referred by Coos County and work collaboratively with a multi-disciplinary team to identify client needs and determine appropriate community resources to resolve identified needs. The role includes providing skills training to promote successful community living for individuals who have high hospital utilization, minimal independent living experience, lack of appropriate health care, and behavioral issues that serve as barriers to independent living. Vocational Training Program For applicants starting as a QMHA-R, ColumbiaCare offers a Vocational Training Program to accelerate training, on-the-job experiential learning, and testing preparation for qualified employees to achieve the 1000 supervised hours required to complete their QMHA-I certification. Completion may occur in as little as six months. This is an optional program. Work Schedule Monday through Friday, 8:30 am – 5:00 pm (Full Time, Day). What You’ll Make $22.00–$26.00 per hour, DOE/Credentials. Additional 5% Language Differential offered for bilingual or multilingual candidates. About the Program Bay Apartments (est. 2007) is a 10‑unit studio apartment project in Coos Bay. These apartments serve transitional housing for Coos County mentally ill adults for up to 18months, helping them move toward independent living. What You’ll Need Must be eligible for Qualified Mental Health Associate (QMHA) registration. Eligibility requires one of the following: Bachelor’s degree in Behavioral Science: Psychology, Counseling, Social Work, Human Development, or a related field; Or, minimum three (3) years of verifiable behavioral health experience working with individuals with behavioral health needs in a supervised setting; Or, an equivalent combination of behavioral science education and satisfactory supplemental behavioral health experience and training. Driving is an essential function. Successful applicants must provide a valid driver’s license within 30 days of hire and maintain a safe driving record. A DHS criminal background check is required. The position requires the ability to frequently talk, listen, use hands and fingers, reach, stand, walk, sit, bend, stoop, climb stairs, lift, carry, push, and pull up to 40 pounds, and occasionally use ladders or perform other physical tasks. Crisis de‑escalation and CPR trainings are required, including hands‑on maneuvers and techniques that may involve lifting and transferring clients.
