La Clinica is seeking a Community Resource Specialist to connect patients with community resources addressing social determinants of health. You will document care in EPIC and follow engagement workflows to meet program standards. The role involves advocating for patients, teaching self-advocacy, and occasionally meeting patients in the community to support access to resources.
