Job Description: Position: RN – Home Health Location: Louisville, KY Coverage: 40214, 40215, 40216, 40258, 40272, 40118 and possibly areas within Metro Louisville Duration: 13 Weeks Contract (Possibilities of extension) Schedule: Monday–Friday | 8:00 AM–4:30 PM Compensation: Local Pay: $55.00 - $57.00/hour (W-2) Plus Mileage reimbursement Mileage Reimbursement: $0.48 /mile (First Patient to the last patient) Weekly Hour Guarantee: 36 - 40 On Call/Weekend Requirement: One weekend on call per month to complete any SOCs and on call visits Job Responsibilities: Conduct OASIS visits for Initial/SOC, Recertification, Resumption of Care (ROC), and Agency Discharge. Provide skilled nursing care to home health patients with various medical needs. Complete accurate and timely documentation in HCHB. Assess patient conditions, provide education, and coordinate care with the healthcare team.
