Seven Healthcare are seeking a Travel RN specializing in Case Management for an exciting opportunity in Martinsville, VA. This role revolves around providing comprehensive care and working in collaboration with healthcare professionals to implement and oversee patient care plans in a fast-paced environment.

Key Responsibilities of the RN – Case Management:
- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Address and reassess cost-efficient, appropriate levels of care
- Coordinate care transitions as needed
- Engage specialty resources to achieve optimal resolution
- Act as patient advocate by protecting privacy and confidentiality issues
Requirements of the RN – Case Management:
- Virginia License (compact license accepted)
- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license
- Case Management certification
- Required skills: Home health, HCHB, Oasis, wound care, wound vac, IV, lab draws
