Job Description
Job Title: Physical Therapist - Home Health

Hours per Shift: 5 x 8s, Monday - Friday, No weekends
Experience: Minimum 1 year of home health experience
License: Required Physical Therapy licensure in state of practice
Certifications: Required Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor


Must-Have:
Must know HCHB and OASIS
Doctor of Physical Therapy (DPT) degree, required if license obtained on or after January 2016, or foreign equivalent


Description:
Plans and conducts segments of treatment programs designed to restore and improve physical, social, and mental functions.
Meets department objectives while serving as co-leader in interdisciplinary approaches to patient groups.
Completes evaluations to ensure quality standards and addresses assessment areas requiring attention.
Uses clinical techniques and tools to establish individualized treatment plans for patients.
Provides therapy interventions using appropriate treatment protocols and techniques.
Establishes individualized care and discharge plans, ensuring communication to appropriate parties.
Completes comprehensive and accurate clinical documentation within established deadlines.
Supervises assigned assistants.
Performs other job-related duties as assigned.
Commits to the safety and wellbeing of associates and patients, requiring vaccinations as a condition of employment.
