Admissions RN - PRN

Overview Continuum Hospice is hiring a Per Diem Compassionate Admissions / Resource Nurse (RN) to provide patient care in and around the Santa Cruz County Area. The position is Per Diem, as needed. Responsibilities Review clinical referral information and respond to requests & inquiries, collaborating with the Clinical Director on eligibility and appropriateness for hospice services. Provide pre‑assessment and admission services to patients referred to the hospice, conducting visits at home or in a healthcare facility as necessary. Communicate with the Intake Department, referring physicians, and the Hospice Medical Director to ensure appropriateness of each referral and obtain required certifications. Apply nursing principles and techniques in patient care, offering emotional support, education, and encouraging participation in the plan of care to meet established goals. Participate in on‑call duties as assigned by the Clinical Director and Program Director. Qualifications Current unencumbered license as a Registered Nurse. Bachelor's degree in nursing from a program accredited by the National League for Nursing, or at least one year of professional nursing experience within the last three years. Hospice or home health experience preferred. Computer proficiency in typing and various programs, including background in EMR. Current CPR certification. Valid driver's license, vehicle insurance, and reliable transportation. Compensation $61.00 - $66.00 per hour (average pay range).
