Responsibilities

As the Wound Care Coordinator (RN), you will:

• Perform comprehensive skin and wound assessments upon admission and as clinically indicated.
• Develop, implement, and evaluate individualized wound care treatment plans.
• Monitor wound healing progress and modify treatment plans based on resident response.
• Coordinate wound care treatments, including dressing changes and specialized therapies.
• Collaborate with physicians, nurse practitioners, therapy staff, dietitians, and the interdisciplinary care team to optimize resident outcomes.
• Monitor residents for signs of infection or complications and communicate significant changes promptly.
• Educate residents, families, and staff on wound prevention, skin integrity, and treatment plans.
• Conduct routine skin rounds and participate in interdisciplinary care plan meetings.
• Ensure compliance with all federal, state, and facility regulations related to wound care, infection prevention, and documentation.
• Maintain accurate and timely documentation in the electronic medical record (EMR).
• Track wound care outcomes and quality metrics to support continuous performance improvement.
• Assist with Quality Assurance and Performance Improvement (QAPI) initiatives.
• Serve as a clinical resource and mentor for nursing staff on wound care best practices.
• Manage wound care supplies and ensure appropriate inventory levels.
• Perform other duties as assigned.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the State of Illinois.
• Wound Care Certification (WCC, WOCN, or willingness to obtain) preferred.
• Minimum of one year of wound care experience preferred.
• Skilled nursing or long-term care experience strongly preferred.
• Strong knowledge of wound care practices, pressure injury prevention, infection control, and regulatory compliance.
• Excellent assessment, critical thinking, communication, and organizational skills.
• Experience with electronic medical records (EMR).

Physical Requirements

• Ability to stand, walk, bend, reach, and lift up to 50 pounds with or without reasonable accommodation.
• Ability to perform resident assessments and wound care treatments throughout the shift.
• Ability to respond promptly to resident care needs.
