Clinical Nurse

The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Applies the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after care. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team. Manages assignments within the care delivery model and demonstrates effective allocation of material resources. Role models patient-centered care by facilitating patient engagement and addressing their needs. Considers patients' values, preferences, cultural diversity, and expressed needs in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and family members who have concerns. Demonstrates caring, respect, compassion, empathy, and active listening. Conducts purposeful hourly rounding on assigned patients addressing their needs. Practices safe and efficient patient-centered care. Demonstrates clinical skills and knowledge crucial to safety in the patient population served. Implements process improvement strategies based on evidence-based practices. Engages in peer and institutional review processes to enhance care quality. Collaborates with interdisciplinary teams to create and implement quality improvement projects. Contributes to quality and safety practices and compliance measures. Implements principles of high reliability to provide a safe environment for patients. Utilizes institutional information technology to document nursing practice and support improvement initiatives. Performs charge nurse duties as assigned. Coordinates with leadership to ensure supplies are appropriately utilized and available for patient care. Escalates resource deficits to appropriate management. Demonstrates patient-focused and cost-effective approaches to care. Documents care accurately and thoroughly to comply with standards. Facilitates patient throughput on shift basis. Advances clinical competence to progress from novice to expert. Projects a professional image and communicates with respect and compassion. Incorporates ethical principles into decision making for patients and families. Encourages a spirit of inquiry and life-long learning for self and others. Participates in shared decision-making through practice councils and unit initiatives.
