A reasonable caseload that allows you to provide exceptional, high quality care while maintaining a good work life balance Hours: Monday-Friday 8:00 a.m. – 5:00 p.m. Holiday requirement, 1 major holiday and 1 minor holiday per year Opportunity for additional shifts Expected to make up to 5 visits per day with allotted time to complete patient documentation Highly autonomous and functions with minimal supervision Weekly interdisciplinary meetings 24/7 support from our full-time Board Certified physicians and APRNs, Chaplains and Social Workers Interdisciplinary team work to support outstanding patient and family care Position Description We are seeking a compassionate Registered Nurse (RN) to join our interdisciplinary hospice team in providing high quality, expert care to our seriously ill patients. Our RN Case Manager is a vital part of our hospice team providing comfort, care, support and education to our patients and their loved ones.
