The Clinical Pharmacy Specialist is also expected to serve as a leader among their colleagues, supporting workflow and education as appropriate.The major job responsibilities include identification, resolution, and prevention of drug-related problems, drug distribution support, order verification, and oversight of medication use protocols and/or restrictions. Infectious Diseases Clinical Pharmacy Specialist serves as a key member of the multidisciplinary infectious diseases and antimicrobial stewardship teams, optimizing antimicrobial therapy, improving patient outcomes, reducing antimicrobial resistance, and supporting institutional quality and safety initiatives. The Clinical Pharmacy Specialist supports and leads clinical pharmacy and medication management initiatives, serves as the pharmacy representative on committees as appropriate, participates in research, and contributes to the advancement of knowledge through the teaching and education of pharmacy students, pharmacy residents, and pharmacy staff, as well as nursing staff and medical staff both internally and externally. The Clinical Pharmacy Specialist serves as a preceptor for APPE students and pharmacy residents. Responsibilities include program/syllabus development and maintenance to achieve program goals and provision of sound academic and clinical training for students and residents. Job Responsibilities: 1) Provide Comprehensive Pharmaceutical Care and Operational Support of Care Areas:Monitor patient specific opportunities for de-escalation, duration optimizationallergy assessment, reduction of unnecessary antimicrobial use.Maintain and grow our Antimicrobial Stewardship Program, Blood Culture Surveillance Program, and Outpatient Parenteral Antimicrobial Therapy (OPAT) Program.Conduct prospective audit and feedback of antimicrobial utilizationProvide pharmacokinetic consultations when appropriateCollaborate closely with microbiology laboratory personnel.Assist in development and review of institutional antibiograms.Prevent, detect, monitor, and document adverse drug reactions and medication eventsCounsel patients/family on discharge/transfer medications as appropriateParticipate in emergencies as appropriateDocumentation of services provided within the pharmacy intervention database and patient medical recordTrack stewardship interventions, outcomes, and quality metrics.Provide medication order review support as appropriate for patientsResolve drug distribution-related issuesCover assigned shifts as determined by departmental needsEnsure adherence to the Joint Commission2) Advancement of Knowledge for Baystate Medical Center and Baystate Health:Serve as the primary preceptor for APPE pharmacy students and Pharmacy residents in their care areas and a backup preceptor for other rotations as appropriatePlan, develop, and implement staff development support for prescribers and nursesFacilitate Clinical Pharmacist development in the specialty care areaDesign and provide presentations, publications, and other informative activities related to patient care and other drug-related topics for internal and external audiencesMeets ASHP/School of Pharmacy requirements for preceptors3) Develop and Support evidence-based drug therapies for patients:Plan, develop, and implement medication-use therapies that are optimal from both a financial and a clinical perspectiveImplement appropriate clinical practice guidelines and care plans through CISCoordinate Pharmacy and Therapeutics category/medication reviews as necessaryDevelop and conduct target drug programs and drug use evaluations as needed and report results to the Pharmacy and Therapeutics CommitteeMonitor the use of high-cost, high-risk medications to ensure consistency with approved criteria4) Participate in Pharmacy-Research Projects:Medication-related patient outcomes researchServe as a residency project mentor for pharmacy residents as appropriate5) Must be able to obtain any applicable BPS certification within 12 months from the date of hire that is relevant to the clinical scope.Education:Doctorate (Required)Certifications:Board Certified Pharmacotherapy Specialist - Board of Pharmacy Specialist, Licensed Pharmacist - OtherCompensationNote: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system.
