Physical Therapist / Rehab Director – Veterans Home (Jackson, LA)

Synergy Care is seeking a dedicated and compassionate Physical Therapist / Rehab Director to join our team at a Veterans Home in Jackson, Louisiana. This is a rewarding opportunity to provide high-quality care to those who have served our country while leading a collaborative rehab team.

Key Responsibilities

Provide exceptional physical therapy services to residents
Develop and implement individualized treatment plans
Oversee and support rehab staff as Director
Collaborate with interdisciplinary team members to ensure optimal patient outcomes
Maintain accurate and timely documentation

Qualifications

Graduate of an accredited Physical Therapy program
Current or eligible for a valid Physical Therapist license in the state of Louisiana
Strong leadership and communication skills
Commitment to patient-centered care and teamwork
