Registered Nurse/Delegating Nurse & Case Manager

Job Description

We are seeking a Registered Nurse/Delegating Nurse/Case Manager to join our team! Come work in one of the most rewarding fields in nursing. Make a difference in the life of an individual who is appreciative and loves their nurse case manager. Join our supportive nursing team, with years of experience. We currently have a position to work with adults with intellectual disabilities at our Frederick, MD location. Position is eligible for full time benefits. Total of 40 hours per week, M-F day shift.

Responsibilities:

Assessments for assigned caseload
Development of and training for Nursing Care Plans
Monitor and report changes in patient symptoms or behavior
Communicate with collaborating physicians or specialists regarding patient care
Delegate and serve as resource to Direct Support Professionals/Certified Medication Technicians
Educate patients/Direct support professionals about health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and medical facilities
Maintain accurate patient medical records
Provide advice and emotional support to patients and their family members

Qualifications:

Valid MD Nursing License or Compact State License
Previous experience with intellectual/developmental disabilities preferred but not required
Ability to build rapport with patients
Strong problem solving and critical thinking skills
Ability to thrive in a case management environment
