Brooklyn-One RN both programs

Riverdale Supportive IRA - Nursing hours 10hrs
6 apartments

Arlington -Nursing hours 5
5 bed Female IRA
5 hours of nursing support weekly

Staten Island -15 nursing hours ( one RN needed )
13 residents between 2 houses Co-ed

AMAP Recertifications
Diabetes Recertifying
Focused assessment PRN
EMAR/ETAR weekly review for omissions and discrepancies
Medication reconciliation
CARE plans/PONS creation
Review of all clinical information ( Labs, consultation forms, discharge summaries. Patient care plans etc.
Acts as a liaison with medical providers PRN. (discharge meeting/treatment planning)
Is available by phone during 9am-5pm and operates in hr role as a clinical lead.
