LPN - Evening Shift

Responsibilities:

• Provide skilled nursing care to residents under the supervision of the Registered Nurse (RN)
• Administer medications, treatments, and therapies as prescribed by physicians
• Assist with daily living activities such as bathing, dressing, and grooming
• Monitor and report changes in residents conditions to the RN
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Maintain accurate and detailed records of patient care

Requirements:

• Ability to lift, transfer, and move patients as needed
