Summary

As an In Home Caregiver at Home Care Transitions, you will play a vital role in delivering compassionate and individualized care to our clients in their homes. This position is essential for ensuring the well-being and comfort of those we serve, helping them maintain their independence and quality of life.

Responsibilities

Provide personal care services including assistance with daily living activities such as bathing, grooming, and meal preparation.
Administer medications as prescribed and monitor clients' health conditions.
Support clients with mobility and physical tasks, including heavy lifting when necessary.
Engage clients in social activities to promote mental well-being and companionship.
Document client progress and report any changes in health or behavior to the supervising nurse.
Assist clients with developmental disabilities by implementing individualized care plans.
Maintain a clean and safe environment for clients at all times.
Communicate effectively with clients, families, and healthcare professionals to ensure coordinated care.
Qualifications

Proven experience in patient care or caregiving roles, preferably in home health or similar settings.
Strong knowledge of medication administration protocols and patient monitoring techniques.
Ability to work effectively with individuals with disabilities or developmental challenges.
Excellent communication skills for interacting with clients and their families.
Compassionate nature with a strong commitment to enhancing the quality of life for others.
Experience in social work or group home settings is a plus but not required.
Physical ability to perform heavy lifting tasks as needed for client support.
