Regularly check in, update plans as needed, and ensure nothing slips through the cracks. Be the Trusted Guide – Anticipate client needs, provide personalized solutions, coordinate care. Be the Compassionate Advocate – Ensure collaboration between healthcare providers and family members. Oversee care quality and compliance. Who are we looking for? Licensed RN with strong medical and assessment skills Ability to navigate the healthcare system & advocate for patients Organized, proactive, and able to work independently Exceptional communication skills A passion for making a difference!
