Summary

We are seeking a compassionate and skilled Nurse to join our team at Candlewood Healthcare & Rehabilitation. In this role, you will play a vital part in delivering high-quality patient care within our communities, contributing to our mission of providing loving care and quality service.

This position comes with a starting negotiable salary of $65,000.00 annually

The typical schedule for this opportunity is Monday through Friday from 9am to 5pm. Start and end times are flexible based on community need. This is a hybrid position working onsite at the community Three (3) days a week and Two (2) days a week remote at a home office. There are no On-Call responsibilities associated with this position.

Responsibilities

• Completed assessments and treatment planning for maximum reimbursement.
• Document medical information accurately and maintain patient records in compliance with regulations.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Monitor patients' conditions and report any changes to the appropriate medical staff.
• Educate patients and their families about health management and wellness strategies.
• Ensure a safe and clean environment for patients by adhering to health and safety protocols.

Requirements

Requirements

• Valid nursing license in the state of Ohio.
• Strong knowledge of physiology and patient care practices.
• Experience in acute care settings preferred.
• Familiarity with medical documentation standards, MDS and ICD-9 coding is a plus.
• Excellent communication skills for effective interaction with patients, families, and team members.
• Ability to work collaboratively in a fast-paced healthcare environment.
